The health visiting service for children in Greater Manchester is changing. The developmental assessment forms for children aged 0 – 2.5 years old have moved online.
The assessment forms were previously completed on paper, but you can now access them via a web browser link or by downloading the GM Early Years App on your smartphone. This will help to ensure children are developing as expected and are provided with additional support when needed.
For parents and carers, it will:
- Make it easy to access and complete forms on your mobile, tablet or computer – no longer will you need to worry about losing or forgetting the paper forms.
- Provide videos and other guidance to support your child’s development.
- Give you online access to your child’s development records whenever you need them.
- Enable your health visitor to share documents with you through the App
The Early Years project is a partnership between the NHS in Greater Manchester, the Greater Manchester Combined Authority (GMCA) and Local Authorities across the city-region.
